Use the above link to document or request delivery of teacher-made test accommodations. Form must be submitted at least 48 hours in advance so that we can schedule a staff member to administer.
Please print a copy of the form above and complete (including student signature) if a student refuses their accommodations. Deliver a copy to the case manager to be referred to in the next review meeting.

- Replace scores from Mastery Quizzes with the score for corresponding objectives on the Unit Test. This rewards student mastery of material through tutorials and remediation, without requiring you to retest quizzes.
- Post grades from unit tests by objective in the Gradebook instead of as 1 Formal Assessment. Parents and students will be able to track their mastery and progress, and you can retest them on the specific objectives that were not mastered instead of the entire test.
- There are many methods for curving grades if you choose to do so in your class. One way is to take the square root of each raw score and multiply by 10. This helps students with lower scores more than higher, as demonstrated by the examples below for a raw score of 75 vs. a raw score of 50.

Although more complex, you can also use a linear curve to distribute grades within a set range of scores.
